Philippine Statistics Authority (PSA) Original Birth Certificate
Good Moral Certificate from last school attended
Latest Report Card with Learner Reference Number (LRN) (at least 1st to 2nd quarter or 1st term grades from the previous school)
For incoming Grade 1
• Early Childhood Development (ECD) Certificate
For SHS Voucher Holder
• Qualified Voucher Applicant (QVA) Certificate
If schooled abroad
• Complete Scholastic Records
Baptismal Certificate
• Or its equivalent for students belonging to other religious denominations.
